Your first visit to Bayshore Dermatology involves a few special steps so that we can get to know you. To understand what to expect, please read through this page. You may preregister with our office by visiting our Patient Portal. Call us or fill out the Quick Connect fields to the right to request a login ID.

Please make sure to fill out both forms above The Authorization Form and Financial Policy they are separate forms. After you have completed the form, please make sure to press the Submit button at the bottom of the form to automatically send us your information. On your next visit to our office, we will have your completed form available for your signature. The security and privacy of your personal data is one of our primary concerns and we have taken every precaution to protect it.

Per the No Surprises Act, the required "Patient Disclosure Notice Form" and "Good Faith Estimate" are below. A message directing you to these froms has been provided to you in the text message you received upon scheduling your visit. If you would like a copy, please download and print the forms.

Mac Users You must open and submit the form in a Safari Browser with the latest Mac operating system. It is also important to have the latest version of Adobe Acrobat Reader for MAC on your computer in order to submit your form to our office correctly, please download the free plug-in from Adobe's web site.
